Question about being a doctor and a specialist?

Let's say you go to college and medical school and become an M.D. If you want to be a specialist like a neurologist or a cardioligist, do you have to go to school longer than an M.D. that practices as a family physician? How does becoming a specialist work? I'm only curious.
